A video and booklet for caregivers offers guidelines on moving and transferring someone at risk for falls, as well as how to avoid injury.

It addresses fall risk factors and interventions, and how to recognize and screen someone at risk, including making appropriate referrals to other health care professionals and community resources.
Fall Risk Assessments
These free screenings offer a thorough assessment of an individual’s risk as well as practical tips and recommendations to reduce the risk of falls.
In the community
Fall risk screenings, including a comprehensive health history, medication review, and balance assessment, are conducted at community sites. Participants are screened for fall risk factors and are given recommendations with written documentation to be shared with their primary physician as appropriate.
